If a Sharing Member under the age of 65, dies after 2 years of continuous monthly membership, financial assistance to the surviving family for loss of life expenses will be provided by the Sharing Members according to the following scale:

Primary Member: Ten thousand dollars ($10,000)

Dependent Spouse: Five thousand dollars ($5,000)

Dependent Child: Three thousand dollars ($3,000)
In order to designate who should receive those funds in the event of a Member's passing, please fill out the Loss of Life Assistance Form.
These funds are intended to be used for funeral/burial expenses, or other costs associated with loss of life. In order to direct who is to receive these funds, you will need to fill out and submit the Loss of Life Assistance Form.
Upon a Member's passing, please send notification with the Member's name, Membership ID number, and a copy of the death certificate to us at info@solidarityhealthshare.org in order to start the process of paying the loss of life assistance.
*If there is no Loss of Life Assistance Form on file, the financial assistance will be paid to the Member's estate, or pursuant to an order of the applicable Court with probate jurisdiction.
